Hours: 8:30 a.m. - 5:00 p.m. Monday - Friday (except state holidays). Receives applications for Cash, SNAP, and Medical Assistance by fax, mail or in person.
You can apply for your Illinois EBT card at this local DHS office. If you already applied for your EBT Card and this is your local office, you will either be asked to come to this location for an interview or participate in a phone interview after your initial application for Foodstamps, SNAP or cash assistance is reviewed.

Apply for EBT Card
Most states allow online applications for EBT Cards through the application for Food Stamps (SNAP Supplemental Nutrition Assistance Program) or cash benefits through Temporary Assistance for Needy Families (TANF). We provided this local office contact for those who want to apply for their EBT in person at this office. After applying for your benefits you will most likely need to come in for an interview at this or another local social service office.
